If you prefer a paper copy to fill out, do one of the following: 1) Pick up a copy from a Transition Team member in the Hospitality Hall after a service, 2) request one by sending an email to survey@napc.org and one will be mailed to you (please include address), or 3) call 404-875-5853 and leave a message (please include name and address) and one will be mailed to you. When complete, drop it in the box labeled Survey Dropbox in the Hospitality Hall, give it to a Transition Team member on or before April 14 or mail to the church with Attention: Transition Team by Monday, April 8.

Please read the following carefully before taking the survey:

1. Please take the survey in one sitting. You are not able to save the survey and return to it later. The survey will average around 30 minutes to complete.

2. When you encounter the word Pastors in a question this refers to Pastors Jasmine, Irene, Glenn, Steve, and Fred Foy in total. So if it asks if your Pastors do something, if one or all do it, you would answer in the affirmative or as the instructions provide.

3. For question 68, 3 years ago was in the middle of the pandemic, answer using 3 to 5 years.

4. For questions 79 through 85, and 91 and 92 it asks that only one person per household fill out the answers (for those not filling it out, there is a choice of someone else answered). You may wish to discuss this with other household members and decide in advance who will answer.

5. For question 99, Church Board means you are currently serving as an elder or deacon.

6. For question 100, please use each number once. Also, if you have traits of a new senior pastor that you wish to include but aren’t listed, include those in the Freeform answer to question 104 or send us a response with your thoughts to survey@napc.org

7. For the last question,104, the answer is freeform to provide input on what NAPC should start doing, stop doing, or continue. You may wish to give this some thought prior to taking the survey. Put anything else you want to comment on, in this freeform field.
